[HSF] Economics and medicine- European style

The article just getting me to a question .... Whether the provision of a
service - be it a medical service- is priced as per the service itself ...
or where it is delivered ... provided that it is the same quality service ?

Hal, Tea  .... in the case of an OPCAB provided in Arkinsas e.g. .... and or
Houston Tx . or Florida .. .. would the price for the provision of the
service vary under the same circumstances and same quality ? based on where
it was provided ?

and if the answer is yes ..... would the price be higher at an area short of
resources -as required expertise, training, logistics-  to provide for the
service ? or should that be the same payment here and there?
