Ben,
  I appreciate your thoughts.  I'm now thinking of getting the  cardiologist 
to PCI the LAD or just treat medically.  I've no idea what his  LV was like at 
the original operation 19 years ago, nor do I have a remote  assessment of 
his LV from the past few years.  The patient hadn't seen a  cardiologist or been 
in the hospital for years.
