No I did not mean "do anything because the patient wants it". I meant we 
can consider an option if he "truly wants it". If he doesn't then there 
is no point pursuing the particular option.

  I've no idea what his stroke volume is, but I suspect it's not  much.  The 
short axis view of the LV on TEE showed a fairly thinned out LV  wall.  If 
there was massive LVH, it might make a difference in decision  making.  I 
disagree with your assertion that the patient has the right to  demand surgery, even 
though you are convinced it's futile.  Personally, I  won't knowingly 
participate in "suicide by surgeon."
