Ani,
  Maybe if the previous surgeon did more LIMA's to the LAD, you would  see 
fewer transplants.  I do agree, in general, to your premise.   There should be 
some objective evidence of remaining at risk viable myocardium  before 
recommending surgery.  If the patient has no angina, then PET  or Persantine-Thallium 
for myocardial viability should be done.
