Can you elaborate on the following questions:
1.Any litterature opposing the myth of  open artery better than close one.
2.What is the real scientific value of the quote"VT shows viable myocardium"
3.Again,is there any scientific value of chest pain showing viable myocardium in severely dilated LV?A senior of mine used to say that in dilated LV(LVED pressure >25 mmHg) the O2 supply decreases due to increased intracavitary pressure and decease in transmyocardial pressure gradient in intramyocardial coronaries.
4.Is there any litterature (RCT) showing the beneficial effect of revascularization in dilated ventricles( not only low EF but grossly dilated LV)
