[HSF] 20 year old year S/P DORV repair, Kawashima type spiral tunnel with 100 mm Hg LVOT gradient

Polling the group, 20 year old girl now 18 years after DORV repair with spiral tunnel and 2 level LVOTO totaling 100 mmHg. Echo shows gradient to be immediately subaortic and at the level of the VSD. What to do?
