Just a gentle jab, we don't extubate on the table unless the tubes are dry,  
and the ICU is around the corner. If we are pressed for time we don't extubate 
 on the table, we leave it to the ICU. I guess the point was one of the  
following, if the case has gone well, and the patient has done well, and they  are 
not old as dirt or as young as a new chick and still confused;  regardless of 
the complexity,  then early extubation is reasonable. We do  not extubate 
anyone hemodynamically compromised, for us that is > thann 0.1  of epi, or 10 of 
dopamine. We do routinely extubate our pulmonary hypertensives  with the 
double patch as they seem to do much better when extubated. See you in  Ft.
