Bill,
  Your extubation policy sounds like what I'd do if I had your set of  
patients.  The point I keep bringing up is that I believe some docs pursue  on table 
extubation simply for bragging rights.  Having to deal with an  airway in a 
seriously bleeding patient is just added risk.
  Hope you have a nice Thanksgiving.  The poor children and  parents in many 
countries are lucky you are around.
