If I criticize you, it is knowingly at my own peril. If I provoke you, it is for the benefit of all. Perhaps there is some old school in me. I doubt you had much depth of malfesence. However, lashes are lashes whatever the intent. whether we like lashes may stray into even forbidden subjects for me.
Forgetting the hemodynamic question, which has a variety of solutions, as you noted in a side comment (my favorite source), what about the longevity issue which has less intraoperative solutions.

Dear Tea,
At my alma mater there is a debate going on between a modern historian and my generation of graduates. He is saying we were taught by a bunch of bullies who believed in humiliation as a form?of pedagogy. We are saying we were not aware of being humiliated; we respected them as dedicated teachers who wanted above all for us to get things right. I take your criticism of the ???'s that pop into my postings as my fault, sir,?and I shall work hard to be sure that it does not happen again, sir.
The newer generations of pericardial valves will have better hemodynamics than the older ones. The importance of this will be in the smallest aortic sizes only. I doubt it will be easy to demonstrate a difference today between the valves we tested 25 years ago.
Bob
PS I have proof read this posting, and there is not a single unwanted ? in it.

Do you have similar opinions on the newer generation valves vs the Mitroflow or
older Edwards and against each other?
tea
Maybe others will be more interested in this this less philosophic question??
btw If I put in more "?" will? I appear ? smarter?? like you and ?Hal?

Around 1980 we tested four valves pericardial valves?in our lab. We needed a
replacement for the Ionescu which we had shown to be subject to abrasion
wear.?The Edwards pericardial and the Mitroflow had the best durability. The
Edwards seemed slightly better and we chose it for the aortic position, but not
for the mitral position.
The hemodynamic performance?of the two valves was similar?.We started the
clinical?trial of the Edwards with several other institutions and when that was
finished, started a trial with the Mitroflow. After 31 cases the Mitroflow
company had to discontinue for commercial/financial reasons and they moved to
Canada. After more than 20 years they are finally getting back to the USA.
As we predicted the durability seems to be similar to the Edwards. In our view
the hemodynamics of both were good but we routinely enlarged the root for sizes
19 and 21 in order to be sure we did not leave patients with gradients that
might not allow optimal relief of the load on the left ventricle. We started
this long before the infelicitous term "Patient Prosthesis Mismatch" was
invented.
Ani I wonder if you are thinking of the Duraflow when you say the Mitroflow had
no struts? The struts are actually quite tall, very similar to the Edwards,
Bob

Just wonder if anybody have experience with Mitroflow aortic valve made by
Sorin. It has been used in Europe for a while. Now they are marketing in US.
Thanks!
