[HSF] Ahh, the Holiday Transfer. Would anyone do anything different?

Did you ever consider treating him medically? I am a really agressive surgeon when it comes to aortic stuff, but the mortality in this situation, particularly if he had had mediatinitis previously is really high. We have treated several pts in the last yr with medical management in the face of acute type 1 dissections and so far have done ok. Of the 3, one came back with severe AI and CHF, several months later, was operated on electively and did fine. One has Ossler Weber Rendeau syndrome and bleeds profusely from every mucosal surface if you look at him wrong and is now almost a yr out and is doing ok. The third died of multi organ failure that was present on admission.
I don't have the reference, but the use of medical management in selected cases is a viable option. Otherwise without being there it is hard to say what I would have done differently.

though about a left chest tube -?
didnt get into the space on opening and was a little scared with the IMA to go looking?
he was huge and they didnt see an effusion on tee - but hard to tell.......?

thought about leaving his chest open - but didnt - dont have a good answer, other than his hemodynamics didnt change too much (well other than the arrest) and I watch him for a while. Took a while to get the chest closed.?

Oh, did I mention they treated him for several weeks after his surgery for "wound infection"..... no other details on that one.......?
