Ani,
  I follow your reasoning about the lavish use of health care dollars  on 
high risk, high tech procedures.  However, I would also tell you that  the single 
most important problem in the undeveloped countries is uncontrolled  
population control.  Families of 16 in Central America have nothing to  eat.  Why?  
What I've stated may be politically incorrect.   However, before there are any 
attacks on my position, please check the  facts.
  From what you know of my practice, you know that it consists of  mainly 
complex valve work.  Less than 10% are stand alone CABG's.   Though I have a few 
people that never have a good recovery, the vast majority of  patients 
definitely benefit from sustained improvement in their cardiac  function.  Doing 3 
valve, 5 grafts, and a maze on an octogenarian is tough  on the patient as well 
as the surgeon, but it is usually worth it.
