Theofilo,
  I believe and sympathize with the ridiculous compensation figures  you 
quoted for your system in Brazil.  A couple of years ago, I was invited  by 
Trinidad's minister of health to visit a public hospital in the Port of  Spain.  
Though I've trained in gritty hospitals such as Cook County and LA  County, that 
hospital in Trinidad was by far the most wretched I had ever  seen.  They 
wanted me to do valve surgery there for about $800 per  case.  Mind you, this is a 
country that is awash in petro dollars.   Unfortunately, the corrupt system 
benefits only a few, and the masses get  screwed.  Very sad.
