Theofilo,
Your system is not socialist which pays a salary  and takes away  
incentive. It is a kind of incentive bases sweat shop system which  
would be fine if they paid a decent rate and I expect it is,   
therefore, open to under the table incentives as in Japan
Incidentally  "communist" China has a totally capitalist medical  
system where the patients get no treatment if they can't pay and  
stuff like heart surgery is very expensive.
