[HSF] angiogram and Denying CABG

John,
  Good God!  If we turned down 20% of the cases referred for CABG  because of 
poor targets, we wouldn't be doing much at all.  I very rarely  turn down 
someone for surgery based on diffuse CAD.  Do you do  endarterectomies?  
Particularly for the LAD, being able to offer  endarterectomy in order to "create" a 
target makes it rare that we can't help  nearly all patients.
