Awareness during anesthesia definitely does occur though is thankfully very rare. It is no fault of the anesthesia given. When I was in UK patient once woke up asking what does "flow up", "flow down" mean. However on balance of probability your patient was not truly awake during the procedure. You should ask her now to describe what specifically she was aware of (you may find some things may have been in the pre-surgery phase or post surgery phase and not truly under anesthesia) and specifically what she has heard. This is important as with coaching she could later come up with a very believable story.
