The patient had an early post-op ventilatory difficulty mostly due to
the most severe emphysema I have ever seen, which added to pulmonary
edema from CPB made me leave the sternum open for two or three days
with only the skin closed, With aggressive diuresis we were able to
close it later and the remainder of the post-op proceeded
uneventfully. He was discharged from hospital and seen early post-op
with normal recovery, but have no knowledge of his status since.
Re: the heparin induced bronchospasm, this was controlled by going on
CPB and vasoconstrictors, while removing the Swan Ganz. The remainder
of the operation was uneventful, but the CPB was longer than normal
for a triple bypass.
