[HSF] Lost leaflet

Dr Frater I witnessed a similar case during my training about 7 years ago.
 
The case was a patient with a St Jude mitral valve having a rereplacement for paravalvar leak (extremely common in London because, I believe of the widespread use of semicontinuous prolene suture technique). In process of extracting the valve the surgeon grasped one of the leaflets with a tonsil (or robert's) clamp. At some point the leaflet dislocated off the hinge mechanism. When we eventually got the valve out we noted one of the discs was now missing. We searched the left atrium, opened the aorta and even did a left ventriculotomy and could not find the rogue leaflet. The patient did okay and till today we do not know where that leaflet ended up. I do not recall whether we got a CT scan.
