Dear Ahmed,
You have to review the whole system of cardiac surgery in your hospital:1.Resterlization process of reusable products must be properly reviewed.
2.Assure the sterlity of the single use products:shelf life and so on.
3.Make sure that the air conditioning and air filteration system works properly specially in operation room.
4.Make sure that the members of the team ( specially operation room and ICU ,where invasive cath.s are still in place) strictly obey to the rules of  surgical infection prevention.
5.If the causative pathogen is the same  organism in all cases consider a carrier in the team.
