Subject: [HSF] Image of the week - sternal infection

Speaking of mediastinitis.
Patient underwent mitral valve replacement 3years ago. Sinus occurred 3
months after surgery. Wire removed and sternum healed. a few months
later another sinus appears and another wire removed. a few months later
comes back again with another sinus - cyanoacrylate glue injection
healed the sinus and all was well again for a year when again it
recurred. This time the sinus was explored and lead to the costochondral
junction and it was excisded. Came back again after some time and this
time a CT scan showed a sequestrum and sinuses. Half of the manubrium
and, costochondral junctions and sinus tracts (after injecting methylene
blue) were excised sequestrum was removed and the defect was closed with
a pectoral flap.
