The 'literature' results are already different because only the experts, academics and those with good results report their series. For example if you read through the literature you will see series where the mortality for reoperative surgery is less than they have for first time surgery. Like I remember the series from Harefield (my home institution) on redo homograft (after a previous homograft) had a mortality below 5%. Pooling data from experts and experienced surgeons provides a skewed view of the real world which is partly why it is very difficult to get the true interpretation of risks and benefits from the literature. Also within those expert practices, they only publish those parts of their practice where they experience good results. For example, if you went through the literature you might find surgeon A's results with OPCAB but see nothing about his mitral repair results and surgeon B who publishes 100 mitral repairs, you see nothing about
his results with arch replacement etc. There is so much publication bias that data on survival in the literature have to be taken with a grain of salt.
