It is also the specific problem of a place or corporation as to your Scandavian example. I bring in partners or work with others out in the boonies or occasional go to town. You train new surgeons to do what? If you haven't noticed most of America is small business or rural. Elites of course dismiss such realities. Capitalism and biology are diverse and messy, complex and awesome. They are, fair enough, my models and my criticism. When i quit or die, it is somebody elses problem. All that will be left is my work and ideas for others to criticize, reformulate, or largely ignore. My PA probably will have no trouble, apparently from this thread, in continuing if she wishes.

Tea, no doubt you with your PA are doing good work. However that is not a
system that can continue when either of you are not present. It is a highly
personalized arrangement. What we need is a system that can produce good care
consistently and which is not dependent on any single individual, a system that
