mis-extubation with coughing on the tube ???
or mis-intubation ??? concealed by the tube ... uncovered when tube was
removed ?? " ?? unlikely?"

an "act" of intensivist ? ... or anaesthetist ??
were the intensivist the reason ? or the savior ? .. or both ?
