Michael,
She was on coumadin .The reason for INR>10 is that the manufactoring firm has  changed the shape and color of coumadin from 5 months ago in this country and from that time on we have  had major bleeding problems in patients on coumadin. The producer has been repeatedly informed by myself in this regard but till this day nothing hasn't happened and I haven't been informed about the results of their investigations.
No ,swan couldn' t be placed .(BTW  she is real fatty) Dopamine used  for diuresis and it works well. 
She is improving now.
