Ben,
  You threw out quite a few provocative statements with this  posting.  
Believe me, I still do plenty of redos with the numerous  referrals I get for 
patients with valve disease and previous CABG.  By  peripherally cannulating and 
starting CPB prior to splitting the chest, most  redos aren't nearly the ordeal 
for me that they were 10 years ago.
  Your suggestion to consider TMR or Vineberg in end stage cases is  
something that wouldn't be accepted by a lot of my colleagues.  I did the  first TMR 
in Broward County in 1998.  I only did a handful of cases, and it  never took 
off.  If you've noticed, there is very little about it in the  recent 
literature.  Before I abandoned its use, I felt a little like a  snake oil salesman.  I 
thought I saw a few dramatic results, but they were  sparse.  One thing I do 
know, TMR is ridiculed by my cardiology  colleagues.  Enough said.
  As for Vineberg, I would be too embarrassed to suggest its  use.  Please 
show me a recent publication justifying this procedure.
