Dear Dr.Frater,
I don't have TEE in the op room , whenever needed I ask the cardiology fellows to bring their own TEE probe.So direct visualization + finger  data and preop TTE  may parallel that of TEE.My approach to mitral valve is mostly trans-septal unless I am in hurry to cut the Xclamp short .And the idea of prevention in TR isn't  well taken by the social security system and cardiology circles here and at the time being.
