Though redo operation through R chest proved to be very difficult and disasterous, sternal re-entry has its own pitfalls.Personally I get thoracic CT before any redo cardiac operation and by studing it very carefully -sometimes with a radiologist-try to stay away from trouble. I always cannulate a peripheral artery before sternotomy and open specially lower half of the sternum with Lebsche knife 1mm/second!This way I have been able to open the chest in almost all of my cases without any cardiac injury ,but I have also witnessed very experienced surgeons injuring the heart very severely sometimes deadly in sternal re-entry.So let's not call a sternal re-entry "a piece of cake.
