Hal,
All your precious recommendations are well taken but when it comes to the hospital adminstration I have to spend the energy of a maraton race  to make a distance of only 100 meters.( hope I could clarify the situation here) . regarding TEE ,this is the 1.st step and when the cardiology people don't give the neccessary support then will come the 2.nd step and that would be the training of anesthsiology people and also - and this is very important- me.
Hope my plan will work.
