We also hava a shortness of resources here in 
Padova and we pose the same questions You have rised.
Personally the single criteria I use in this 
setting ( but the decisions are always discussed 
and collegially decided among our group !)  is if 
the patient has symptoms that render his life 
unacceptable (as for instance rest angina 
paroxismal dyspnea) and can be fixed.
