As Ani pointed out about SVR, transplant centers are (not completely) marketing tools for patients that need bread and butter surgery all be it in more complex patients. And what is wrong about that, or the desire to fix them before the get there?

I see those type of patients fairly frequently, as I'm sure do most
members of HSF. I suspect your cardiologist is a bit too aggressive with the
transplant referral reflex. For one thing, what is the likelihood that the
patient will actually receive a donor heart? Perhaps Ani or Michael could address
that question. In my own experience, only a few of the patients sent for
transplant actually get a heart.
Anyway, sending the patient to transplant with an EF of 25% and good
targets and a negative viability study begs the question as to why the patient was
having angina and EKG changes. Doesn't sound like a dead heart to me.
