[HSF] Access to AV groove area post bypass

Open the right pleura, cut the pericardium back to the right phrenic nerve  
and roll the heart under the right side of the sternum. Sutures in the  
pericardium near the left pulmonary veins for upward traction may help. This  
exposure makes anastomosis to the circumflex in the a-v groove relatively easy.  John
