[HSF] type III dissection

62 y/o male diagnosed as type III AD and transferred to our hospital. BMI > 40% + severe COPD.CT showed false lumen from proximal arch  up to mid abdominal level.On beloc,Ca blockers and niprus for the last 48 hours . BP 110/80 in L radial artery and 85/60 in R femoral artery.No pain,no organ or peripheral malperfusion. Hemodynamics stable. Neurologically intact.Control Ct this evening showed moderate fluid in the L hemithorax.Proceed with surgery or continue medical therapy?
