[HSF] Indications for maze with mitral surgery

There may be one difference I don't "cook" with the probes but set up
an arc which is based on its cutting off as soon as dehydration occurs
and so is self regulating. Anyway I havent had an issue after burning
over 600 cases !! The methodolgy used are different.
