Hal,
You make an excellent point and it is definitely a weakness of the  
paper.
The problem is that there are  a lot of conversions because it  
includes all our surgeons and their indications and methods are  
variable.
One guy for instance is a fair weather sailor and rarely does circs  
off pump, so if it looks difficult when the heart is flipped he does  
the  case on pump with a X clamp ~ 30%  whereas I do 5% on pump,  
electively, beating heart. Urgent crash conversions are are not a  
feature of our practice because of our caution and experience.   
However, since most of the cases are those of the two dedicated  
opcabers who eschew the aorta if possible  I think the conclusions  
are close to valid. The anaortic neurological complication rate is  
the  same as that for coronary angiography!
