One of last evening I was called for acute tamponade following the  
removal of a temporary pace-maker lead; a cardiologist tried to  
evacuate the blood percutaneously in another hospital without  
success, despite the removal of more than one liter of blood. You can  
easily see why the patient conditions worsened after the drainage....  
the catheter was into the right ventricle! (at least 8 cm). The  
patient was in severe shock (40 mmHg systolic pressure, high  
inotropes etc.) Now she is doing very fine and once again I am  
surprised of the importance of being lucky
