Hal,

Two years ago the intensivists ran around making sure everyone with two elevated glucose readings was on an insulin drip.
Nowadays the same doctors are running around taking patients *off* inappropriate insulin drips.
The wave, you know.  

Sometimes I'm pretty sure that pricking the patient's finger every hour all night long is enough to raise his stress and glucose levels to ensure he "needs" a unit an hour IV.  ;)
Also, when patients get insulin infusions, we must ensure they have a source of glucose, which can mean that they now get D5W so they can have an insulin drip.  
I look forward when we have a more reasonable and sensible approach to glucose control in place.  :)
