[HSF] Warfarin for rings

No coumadin for rings or prosthetic valves. It took a while to convince
cardiology this was okay as the ACC guidelines suggest coumadin for a few
months. Many of our patients are from rural communities so not having them on
coumadin reduces bloody effusions without any increase in post op CNS events.
