Bob,
I think you have reached your goal (or closer to that goal than many  
of us will ever come) - I and I am sure many others look to you as a  
role model.  Unfortunately the system which you describe is how many  
medical systems are being run - particularly now that "doctors" are  
relegating much of day to day patient care and management to other  
"health care providers"
