As i have said despite both of us being community surgeons in satellite towns, we seem to have different populations of patients. Most of your patients are not sent for SVR, but valvular problems. Not surprisingly you associate SVR with MR which is likely the norm for you. While I also see MR as do you CAD most of my patients are primarily sent for CAB, so MR is not a dominate finding in my SVR volume (which may not be sent for SVR either by the way). In Menicanti's series mitral repair or replacement was necessary in about 15%. As to your direct question if the MR is mild it is often improved with SVR which has been my experience also. To date I have just added a transventricular suture annuloplasty as does Menticanti when necessary.
