There was another interesting report 
of  Hetzer  "Succesful correction of aortic valve 
incompetence and ascending aortic aneurysm by 
relocating aortic valve plane through short aortic graft replacement " C77.
He says that the cranial relocation of the plane 
of the aortic annulus  takes care of grade 3-4 aortic incompetence.
This is a very provocative observation !
Our center has presented a poster (P 127) on the 
techniques to replace the aorta and the non 
coronary sinus by reshaping the prosthesis to form a non coronary sinus.
Altough been in the poster I do not use this 
technique because, as Hetzer, I think that 
displacing  cranially the non coronary annulus 
reconstitutes the correct simmetry of the annulus 
displaced by the aneurismal dilatation. But i 
don't have data on the effect of this on aortic 
incompetence. (we would have replaced most valves with 3-4 incompetence).
Someone has experience or data to comment.
